PATIENT PORTAL FAQs
SETTING UP THE PORTAL (First Time Users ONLY)
If you have not yet setup your Patient Portal Account, you will need to select “Activate Account” button on the Portal Sign on Page. You will have to input the following fields, identically to how they appear in your portal sign up letter.

- Last Name
- First Name
- Date of Birth (must be in MM/DD/YYYY format. We recommend using the calendar option and selecting the date from there).
- Email Address
- Confirm Email Address
Once you have input that information, you will need to select three different security questions and supply their answers. Chose something you will remember since these are unique to you!
(NOTE: You cannot have two answers be exactly the same as each other)
Lastly, you will need to create your Login Information. Your Login or Username will be only Letters or Numbers and cannot contain a special symbol.
The Password must be input twice and must contain at least 8 characters, one UPPER
CASE LETTER, one lower case letter, one number, and one symbol from the following list (~!@#$%^&*-=+_~?/><[]{}|\), and cannot include any part of their name or simple words.
You will then need to supply the 8-digit code provide by the office or your Activation Letter. If you do not have your activation code the office will be glad to supply you a new one.
FORGOTTEN CREDENTIALS
If you forgot your Patient Portal Password or Login Name, no need to worry. You can first attempt to click on the Forgot Login or Forgot Password options on the patient portal
For Forgot Login you will need to supply your email address and you will be prompted to fill out a semi activation form once again. You will need a NEW activation code so feel free to call the office at (856) 424-5005 ext 1281.
For Password Resets, you will need to first select “Forgot Password” instead of “Forgot Login”. You will then be prompted to input your login name and email address. Following this it will ask you for either one, or two of your security question answers in order to reset your password.
Once successfully submitted an email will be sent to your email from either “Trabecular Medical Group LLC” or “Medent” containing the new password. This password in their email will be your new password until you then change it under “Account Information” while inside the portal.
HOW DO I NAVIGATE THE PORTAL?
The options are the same on both mobile and PC/Desktop/Laptop. The
options are:

- Appointments
- Medications
- Allergies
- Results
- History
- Chart
- Account Info
- Statements
- Messages
Most of the time any lab results are sent to results tab but can also appear in messages and chart options as well.
The Mobile version menu options are hidden under the MENU tab in the upper left hand corner.
